Abstract

Background: Hyperbilirubin is defined as a total serum bilirubin level ≥5 mg/dL (86 μmol/L). Hyperbilirubinemia is a transient condition that is often found in both term babies (50-70%) and premature babies (80-90%). The first aid for hyperbilirubinemia cases currently is phototherapy, albumin transfusion, and liver protection. Phototherapy has several side effects such as diarrhea, dehydration, skin rashes, retinal disorders, hyperthermia, and others. Baby massage therapy can be implemented as an additional therapy to phototherapy in reducing serum bilirubin levels in neonates. Objective: Describing the results of pediatric nursing practice that focuses on nursing care for neonates with hyperbilirubinemia based on the review of evidence-based learning. Method: The method used is a qualitative description with a case study approach and journal review of neonatal patients with hyperbilirubinemia. Results: The results of the assessment showed that the three treated patients looked yellow, the icteric sclera, the yellow mucosa, and the total bilirubin was > 5 mg/dL. 10 nursing problems were found in three treated patients with neonatal icterus as the main nursing problem. The act of massage therapy as an additional therapy to phototherapy in overcoming the problem of neonatal icterus affects changes in the patient's serum bilirubin levels. Discussion: Baby massage can improve lymph flow and blood circulation. This increase in blood circulation can cause bilirubin isomers to be excreted more quickly in the feces, therefore increasing bilirubin excretion. Massage therapy intervention is carried out twice a day at the beginning of the service and the end of the service for 15-20 minutes. Conclusion: Massage therapy as an additional therapy to phototherapy reduced the patient's serum bilirubin levels. In conclusion, massage therapy intervention as an additional therapy to phototherapy can be an alternative in reducing serum bilirubin levels in patients with hyperbilirubinemia.
