Abstract
Urea plant of PT. Pusri Palembang is a factory that produces urea using rawammonia and carbondioxide gas supplied from the ammonia plant. The process of making urea is divided into six units. Synthesis section is the first unit in the process of making urea, which is the process uses high temperature and pressures, so that the risk of failure can occur at any time. The aim of this study is to identify, risks assessment and risk evaluate. This study used qualitative design using the Hazard and Opeability Study (HAZOPS). The information collected by depth interviews, observation sheet and document review. There are several deviation in the risks identification of Synthesis section in each component namely no flow, less flow, more flow, low pressure, high pressure, low temperature and high temperature that has the potential damage to the engine, heat radiation, explosion, leakage, corrosion, fire, steam and turbine damage. And the results of the risk assessment have several instruments that have extreme risk and high risk with the highest category, namely the carbamate condenser component. The urea synthesis section on average high risk and extreme risk that cannot be tolerated because of the potential for explosions or fires at startup due to leakage on the flange or pipeline so that the urea plant must be shutdown , but extreme risk can still be overcome by installing alarm for safeguard or pipeline, engine maintenance, providing of personal protective equipment and safety control.
