Abstract

Background: Klebsiella pneumoniae and Escherichia coli are Gram-negative bacteria from the Enterobacteriaceae family which cause many infections, both hospital-acquired infections (HAIs) and community-acquired infections. The prevalence of resistance to these bacteria is increasing, including resistance to carbapenems and broad-spectrum cephalosporins. Resistance to antibiotics complicates therapy and increases mortality. This study aims to determine the sensitivity pattern of K. pneumoniae and E. coli producing ESBL and carbapenem resistance to antibiotics. Method: This research is a descriptive observational study with a cross-sectional design using laboratory data and medical record data period of July 2022-June 2023 at the Central Laboratory Installation and Medical Record Installation of RSUP Dr. Mohammad Hoesin Palembang which met the inclusion criteria and do not meet the exclusion criteria. Data were analyzed using Microsoft Office Excel 2019 and the IBM Statistical Package for Social Sciences (SPSS) version 22 statistical program and presented in the form of tables and narratives. Result: There were 587 K. pneumoniae isolates and 506 E. coli isolates producing ESBL or carbapenem-resistant. Prevalence of K. pneumoniae and E. coli in RSUP Dr. Mohammad Hosein Palembang, namely ESBL-K. pneumoniae (53.6%), ESBL-E. coli (72.1%), CRE-K. pneumoniae (8%), CRE-E. coli (1.8%), ESBL-CRE-K. pneumoniae (2%), and ESBL-CRE-E. coli (1%). Based on the susceptibility pattern, ESBL-producing isolates were sensitive to antibiotics from the carbapenem group, amikacin, nitrofurantoin, and tigecycline. The CRE isolate was sensitive to amikacin, nitrofurantoin, and tigecycline. ESBL-CRE isolates were sensitive to amikacin and tigecycline. Conclusion: The most prevalent ESBL-producing or carbapenem-resistant Enterobacteriaceae isolates in RSUP Dr. Mohammad Hoesin Palembang for the period July 2022-June 2023 was ESBL-E. coli (72.1%), which was sensitive to piperacillin/tazobactam, carbapenems, amikacin, nitrofurantoin, and tigecycline.
