Abstract

Background: The causative germ of diabetic foot infections are highly diverse and are usually treated with antibiotics. Inappropriate use of antibiotics can influence germs to become resistant and result in increased severity and mortality. Study on the distribution of germs and their sensitivity is rarely conducted in Palembang. Some studies only identify 1 type of germ so it does not describe the overall distribution. The purpose of this study was to identify germ and antibiotic susceptibility patterns in DM type 2 patiens with diabetic foot ulcers at the Inpatient Installation of RSUP Dr. Mohammad Hoesin Palembang. Methods: This study was descriptive observational with total sampling. The data used were medical record and resistance culture laboratory results with specimen post-debridement secretion swabs of diabetic foot ulcer in type 2 diabetes mellitus patients who underwent hospitalization in the period January 2021–December 2022. Results: There were 102 germ isolates obtained from resistance cultures of 76 samples. Gram-negative strains were the most common type of germ (86.3%) which placed Acinetobacter baumannii (19.6%) as the highest germ. Gram-positive germs (13.7%) were dominated by Staphylococcus spp (12.7%). Acinetobacter baumannii was sensitive to tigecycline (90%) and were resistant to ampicillin (94.1%), ceftriaxone (94.1%), and cefazolin (94.1%). Staphylococcus spp were sensitive to linezolid (100%), nitrofurantoin (100%), quinupristin/dalfopristin (100%), rifampicin (100%), tigecycline (100%) and resistant to benzylpenicillin (100%). Conclusion: Almost all Gram-negative isolates were sensitive to aminoglycoside antibiotics (amikacin) and resistant to cefazolin. Gram-positive isolates were sensitive to linezolid, nitrofurantoin, quinupristin/dalfopristin, tigecycline and entirely resistance to benzylpenicillin.
