Abstract
Blood donation is a vital part of health services around the world. The minimum need for blood in Indonesia is 5.1 million blood bags, currently blood production only meets 4.1 million, which means it is still below the needs of Indonesian people every year. To fulfill blood needs, criteria and a national blood transfusion system have been established in accordance with Minister of Health Regulation Number 91 of 2015. Blood donation goes through various stages during donation to ensure the safety of donors and recipients and maintain the quality of the blood. In its implementation, there are various obstacles that result in a high rate of blood donation failures which can hamper the fulfillment of blood stocks in Indonesia. The prevalence of blood donation failure varies from 1% to 67%. Blood donation failure itself can be prevented if the reason is known. This research was conducted to determine the causes of blood donation failure at one of the PMI Blood Donation Units in South Sumatra Province, a fairly large blood donation center in Palembang. Method: This research was conducted at the PMI Blood Donation Unit, South Sumatra Province. This type of research is descriptive observational. There were 3,074 samples obtained in the form of blood donor questionnaires. The data was then processed using SPSS version 27 and Microsoft Excel 2020. This study reported that there were 3,144 donor deferrals (26.2%) consisting of 2 stages, namely 2,691 failures during pre-donation examination and 453 failures during blood collection. Donor deferrals is dominated by age 25 - 44 years, male, voluntary donor type, first time donor status. The highest cause of deferrals was low hemoglobin levels (29.5%), followed by a history of medication (12.2%), high blood pressure (10.9%), donor with vasovageal reaction or adverse reaction (10.5%), low blood pressure (8.5 %), indication of risky activities (7.6%), bad donor appearance (sleepiness and fatigue) (7.5%), high hemoglobin level (5.3%), low body weight (4.5%), failed blood collection (phlebotomy) (4%), and the last cause was a history of disease (1.7%). There were 3,144 donor deferrals (26.2%).
