Abstract

Background: Thalassemia is a genetic condition caused by mutations in the globin gene, leading to a decrease or absence of alpha globin chains in alpha thalassemia or beta globin chains in beta thalassemia. This results in suboptimal formation of red blood cells, leading to anemia in patients, necessitating blood transfusions. Regular blood transfusions result in an excess of iron in the body. Excessive iron in the liver can lead to damage and elevate levels of SGOT and SGPT. This study aims to determine the relationship between the frequency of blood transfusions and liver function in pediatric patients with thalassemia major receiving blood transfusions at Dr. Mohammad Hoesin General Hospital in Palembang in 2022. Methods: This study used an observational analytic method with a cross sectional approach conducted by collecting secondary data from patient medical records. Samples were taken with consecutive sampling technique. The obtained data is analyzed through univariate and bivariate analysis. Results: The obtained sample consists of 85 patients, with the majority being aged ≥10 years, accounting for 69 patients (81.2%). Among them, 49 patients (57.6%) are female, 72 patients (84.7%) receive ≥10 blood transfusions per year, 56 patients (65.9%) have hemoglobin levels in the moderate anemia category, 72 patients (84.7%) have ferritin levels >2500 μg/L, 50 patients (58.8%) have abnormal SGOT levels, and 44 patients (51.8%) have abnormal SGPT levels. Based on the analysis, the frequency of blood transfusions does not have a significant relationship with SGOT levels (p = 0.829; PR = 0.875; CI 95% = 0.261-2.939) and SGPT levels (p = 0.870; PR = 0.906; CI 95% = 0.277-2.962). Conclusion: There is an insignificant relationship between the frequency of blood transfusions and liver function in pediatric patients with major thalassemia.
