Abstract

Background: Fever neutropenia is one of the serious complications of chemotherapy in malignant patients. One of the main causes of febrile neutropenia is a bacterial infection. Early administration of appropriate antibiotics greatly affects the prognosis of patients. The selection of first-line antibiotic therapy for the management of febrile neutropenia varies depending on the local antibiotic in each institution. The purpose of this study is to determine the antibiotic susceptibility patterns to pathogens that cause bacteremia in malignant pediatric patients accompanied by neutropenia fever at RSMH Palembang in the period 2020 – 2022. Methods: This study is an observational descriptive study using secondary data from the Medical Record Installation and RSMH Central Laboratory Installation for the period 2020 – 2022. Results: The prevalence of neutropenia fever for the period 2020 – 2022 was found at 19.1% with 366 episodes of neutropenia fever dominated by men (56.3%) and age group 2 - <6 years (35%) and the most diagnoses in ALL (49.2%). The bacterial group is dominated by Gram negative bacteria (52.5%) with the most isolates from the Gram negative group was Klebsiella pneumoniae (12.7%) and Gram positive was CoNS (38.1%). Klebsiella pneumoniae isolates are 100% resistant to ampicillin and 100% sensitive to tigecycline while CoNS isolates have the highest resistance to benzylpenicillin (100%) and most sensitive to linezolid and tigecycline (100%). Conclusion: The prevalence of neutropenia fever in this study was 19.1% with CoNS as the most common germ. Benzylpenicillin is the most resistant antibiotic to CoNS isolates.
