Abstract

Background: Delayed management of vascular compromise in free flap procedures can escalate the risk of ischemia and necrosis. Previous research has proven that monitoring free flaps through lactate and glucose measurements is accurate in assessing vascular compromise. Therefore, clinical monitoring combined with lactate and glucose measurements is expected to expedite the diagnosis of vascular compromise in free flaps. Method: Patients undergoing free flap reconstruction in the Plastic Reconstructive and Aesthetic Surgery Division at Dr. Mohammad Hoesin Palembang Hospital from November 2022—November 2023. Results: The study involved 19 patients undergoing free flap reconstruction, with the highest distribution in the age group of 19-44 years (47.4%), male gender (63.2%), tumor diagnosis (63.2%), and reconstruction using ALT (57.9%). Viable free flaps showed lower lactate levels and higher glucose levels compared to flaps with vascular compromise. The transition of lactate levels in viable free flaps indicated an increase immediately after anastomosis, gradually decreasing afterward. Lactate levels in flaps with vascular compromise increased above reference value. Viable free flaps showed fluctuating transitions in glucose levels, mostly above reference value. Glucose levels in flaps with vascular compromise decreased below reference value. Viable free flaps were the most frequent outcome at each examination time. Conclusion: Lactate and glucose measurements, combined with clinical monitoring, can expedite the diagnosis of vascular compromise in free flaps, potentially enhancing flap success and yielding better patient outcomes.
