Abstract

Background: Chronic Kidney Disease (CKD) is the deterioration of the kidneys. Kidney replacement therapy is required, one of which is hemodialysis. AV shunt is the gold standard for hemodialysis. This study aims to determine the incidence of complications of AV shunt use in hemodialysis patients at Dr. Mohammad Hoesin Hospital Palembang. Methods: This type of research is descriptive observational from primary data of patients of Vascular Surgery Polyclinic and Hemodialysis Installation and secondary data of patients from Medical Record Installation of CKD patients who experienced complications of AV shunt at Dr. Mohammad Hoesin Hospital Palembang. Results: The study found 45 patients with an average age of 52.58 years, minimum age of 26 years, and maximum age of 71 years. Gender was dominated by men (51.1%). The most common comorbidities found was hypertension (57.8%). The majority had used AV shunt for <1 year (62.2%). The most frequently used cannulation technique was constant-site (53.3%). The type of AV shunt access was dominated by brachialcephalic fistula (84.4%). Complications of AV shunt use were stenosis (57.8%), thrombosis (15.6%), aneurysm/pseudoaneurysm (31.1%), steal syndrome (15.6%), infection (4.4%) and hematoma (6.7%). Conclusion: The study found 45 patients with an average age of 52.58 years, with a minimum age of 26 years, and a maximum age of 71 years. Gender is dominated by men. The most common comorbidities found was hypertension. The majority had used AV shunts for <1 year. The cannulation technique that was often used was constant-site. The type of AV shunt access was dominated by brachialcephalic fistula. The most common complication of AV shunt use in CKD patients was stenosis, followed by aneurysm/pseudoaneurysm, thrombosis and steal syndrome.
