Abstract

Nurses must make diagnoses, outcomes, and interventions based on standards, but in fact, there are still many diagnoses, outcomes, and interventions that are not in accordance with standards. This study aims to determine the application of diagnosis, outcomes, and interventions at Prabumulih City Hospital. This research design uses the descriptive observational method. The sample in this study was 378 medical records of patients who had been hospitalized at the Prabumulih City Regional General Hospital in the period January 2022–December 2022. Sampling was done by the probability sampling method with a simple random sampling technique. Data collection uses instruments in the form of observation sheets. The results of the study found that the application of nursing problem formulation in problem-focused diagnoses was more in accordance with NANDA standards (57.31%) and more risk diagnoses were in accordance with NANDA standards (65.12%). The application of related factors in problem-focused diagnoses was more than according to NANDA standards (69.25%). The application of defining characteristics in problem-focused diagnosis was not in accordance with NANDA standards (54.63%). The application of risk factors in risk diagnoses was not in accordance with NANDA standards (74.42%). The application of outcome labels was not in accordance with the NOC standard (58.99%). The application of outcome criteria was not in accordance with NOC standards (60.85%). The application of intervention labels was not in accordance with NIC standards (55.29%). The application of nursing actions was not in accordance with NIC standards (56.35%). Based on the results of this study, special attention is needed from the hospital to nurses to improve their knowledge of the application of diagnoses, outcomes, and interventions in accordance with NANDA, NOC, and NIC standards.
