Abstract
Introduction: The prevalence of Autoimmune Hemolytic Anemia (AIHA) in pediatric patients with transfusion-dependent thalassemia, including analysis of the risk factors that influence it, still requires further research to prevent delays in treatment and improve the prognosis of thalassemia patients with AIHA. This study aimed to determine and analyze what risk factors influence the occurrence of AIHA in transfusion-dependent thalassemia patients. Method: This research is an analytical observational study of transfusion-dependent thalassemia children at the pediatric department of RSUP dr. Mohammad Hoesin Palembang, with a cross-sectional design to determine the prevalence of AIHA and case-control to determine the risk factors for AIHA in transfusion-dependent thalassemia. Results: There were 140 transfusion-dependent thalassemia patients seeking treatment during the study period, of which 29 (20.7%) were diagnosed with AIHA and included in the case group. A total of 61 subjects were randomly selected as the control group. A total of 65.5% of the case group were women with an OR value of 0.620 (p=0.426). There were only 2 (6.9%) subjects in the control group who had blood type AB with an OR value of 1.473 (p=0.999). There was no relationship between a family history of autoimmune disease and splenectomy with the incidence of AIHA in transfusion-dependent thalassemia patients with OR values of 6.25 and p = 0.999, respectively. The majority of subjects in the case group (82.8%) received their first transfusion when they were ≥12 months old, with an OR value of 1.430 (p=0.731). A total of 65.5% of the case group received ≥72 transfusions with an OR value of 2.097 (p=0.170). A total of 86.2% of the case group had no allergic reactions with an OR value of 1.693 (p=0.573). There was no relationship between a history of parvovirus B19 infection and the incidence of AHAI in thalassemia patients with an OR value of 0.831 (p=0.763). Conclusion: The prevalence of AIHA in pediatric patients with transfusion-dependent thalassemia at RSUP dr. Mohammad Hoesin Palembang is 20.7%. There was no relationship between gender, age at first transfusion, blood type, family history of autoimmune disease, history of splenectomy, frequency of transfusion, history of allergic reactions, parvovirus B19 infection, and the incidence of AIHA in transfusion-dependent thalassemia patients at RSUP dr. Mohammad Hoesin Palembang.
