Abstract

Background. Acute kidney injury (AKI) is a condition of the kidney's that inability to regulate fluid and electrolyte balance which is characterized by a decreased glomerular filtration rate (GFR), which occurs suddenly, and can return to normal. Acute kidney injury is one of the most common complications found in critically ill children and affects the prognosis of patients in the pediatric intensive care unit (PICU). There are several risk factors associated with the incidence of AKI in the PICU, including underlying disease, previous treatment, changes in the patient's medical condition, treatment received in the PICU, age, body weight, use of vasopressors, diuretic drip, mechanical ventilation, illness severity score (PRISM, PELOD 2), heart disease, sepsis, positive fluid balance, negative fluid balance, and nephrotoxic drugs. AKI events in the PICU can occur in early onset (≤ 48 hours) after admission to the PICU or late onset (≥ 48 hours) after admission. The aim of this study was to prospectively examine the incidence of AKI and the factors that influence it based on the onset of AKI. Method. This study was an observational analytic with a prospective cohort design. The samples in this study were pediatric patients treated in the PICU of RSUP Dr. Mohammad Hoesin Palembang who met the inclusion and exclusion criteria for the period 1 September 2023 to 31 January 2024. Results. The incidence of AKI in the PICU of Dr. Mohammad Hoesin Palembang was 22/180 (12.2%), early onset AKI was 16/22 (72.8%), late onset AKI was 6/22 (27.2%). There are 4 factors that have the most influence on early onset AKI, three factors, namely age < 5 years (P 0.041), Pelod-2 score (P 0.013), and oliguria (0.002) as risk factors and 1 factor, namely post-operative emergency (0.070 IK 0.008-0.589) as a protective factor. Pelod-2 score (P 0.011) was the single most influential risk factor for late-onset AKI. Conclusion. There was a relationship between age, Pelod-2 score, oliguria, and postoperative emergency with early onset AKI. There is a relationship between the Pelod-2 score and late onset AKI. Keywords.
