Abstract

Background: Open-heart surgeries such as Coronary Artery Bypass Graft (CABG) involve the use of cardiopulmonary bypass (CPB) machines, which can lead to postoperative complications such as systemic inflammatory response syndrome (SIRS) and myocardial damage. Myocardial protection can be achieved through topical hypothermia by lowering the heart's temperature using a cold solution or ice. Although some studies suggest its benefits in protecting the myocardium, the use of topical hypothermia remains controversial due to its uneven cooling effects and the risk of tissue injury. Aim: To evaluate the relationship between the use of topical ice water hypothermia and pulmonary complications in open-heart surgery. Methods: This study is a retrospective analytical cohort observational study with a cross-sectional approach comparing postoperative pulmonary complications in patients undergoing open-heart surgery who received only systemic cardioplegia versus those receiving a combination of systemic cardioplegia and topical hypothermia. Data collection was performed using consecutive sampling techniques. Results: Out of 32 study subjects, the analysis results indicate that the use of topical hypothermia does not have a statistically significant relationship (p > 0.05) with the incidence of leukocytosis, pulmonary edema, duration of surgery, duration of ventilator use, length of hospital stay, and mortality compared to the group receiving only systemic cardioplegia. There is a significant relationship (p < 0.05) between the incidence of pleural effusion in the topical hypothermia group. Conclusion: The use of topical hypothermia in heart surgery is significantly associated with a higher incidence of pleural effusion.
