Abstract
Background: Chest trauma is the third most common cause of mortality due to trauma worldwide, with a mortality rate reaching 60%. There are several chest trauma assessment scores, one of them is the Chest Trauma Score (CTS). This score can predict patient complications and mortality. Acute Respiratory Distress Syndrome is one of the complications. Therefore, this study aims to determine the relationship and assesss the accuracy of CTS as a predictor of ARDS in patients with blunt chest trauma. Methods: A total of 32 medical records of patients with blunt chest trauma were analyzed. Subjects were assessed for CTS, with components including age, number of rib fractures, presence of bilateral rib fractures, and lung contusion. The data were divided into two groups, CTS<5 and CTS≥5. Data were analyzed to determine the relationship between CTS and the final outcome, ARDS. Results: The results of this study showed 62,5% of subjects had CTS≥5 and 37,5% had CTS<5. A total of 68,8% subjects with blunt chest trauma developed ARDS. Analysis using Fischer Exact Test showed a significant (p<0,05) between CTS values and ARDS in patients with blunt chest trauma. The sensitivity of this study is 77,3%, specificity is 70%, positive predictive value 85%, an negative predictive value is 58,3%. Conclusion: Statistically, there is a significant relationship between the chest trauma vi vii score and ARDS, thus the chest trauma score is considered accurate as a predictor of ARDS in patients with blunt chest trauma.
