Abstract

Postnatal care is an efforts to prevent and manage postpartum hemorrhage and infection. According to the 2017 Indonesian Demographic and Health Survey (IDHS 2017), the proportion of postnatal care utilization in rural is lower than in urban areas. This study analyzes the determinants of postnatal care utilization in rural Indonesia. The study design was cross-sectional using the 2017 IDHS data. The study sample consisted of 7,550 woman of childbearing age (15-49 years). Data were analyzed univariate, bivariate using chi-square test, and multivariate using multiple logistic regression test. The results showed that 75.9% of respondents utilized postnatal care. Education level, parity, economic status, region, birth attendant, perceived distance to health facilities, number of antenatal care (ANC) visits, and cesarean delivery had a significant relationship with postnatal care utilization (p-value <0.05). The birth attendant was the most dominant factor affecting postnatal care utilization (p-value<0.001; Prevalence Ratio (PR) = 4.961; 95% Confidence Ratio (CI) = 3.815 - 6.452) after controlling for other variables. Interestingly, this study found that respondents who gave birth in a health facility were less likely to utilize postnatal care than those who gave birth in a non-health facility. However, the role of delivery assistance by health workers is crucial to increase the utilization of postnatal care regardless of the place of delivery. Health workers not only assist in delivery, but also in encouraging and facilitating postnatal care, both in health facilities and home visits. Therefore, the government is expected to increase the capacity and active role of health workers in providing postnatal care.
