Abstract

PT Pertamina EP Pendopo Field is a state-owned company engaged in oil, gas, and renewable energy. Oil and gas are the world's main sources of fuel consumption. Most of the oil and gas being transport from one location to another through pipelines. Pipeline integrity is of primary interest to oil and gas companies, government agencies, consumer, and other stakeholders due to the adverse potential consequences related to public health, safety, and substantial losses in the event of a system failure. The purpose of this research was to analyze and identify the root cause of crude oil pipeline leakage at PT Pertamina EP Pendopo Field using the Fault Tree Analysis (FTA) method. Qualitative approach by analyzing the Fault Tree Analysis (FTA) method for this research. Information was obtained thro`ugh in-depth interviews with expert informants, key informants,observation, and document review. The results showed that throughout 2023 there were 57 crude oil pipeline leaks with the main cause being external factors in the form of corrosion and damage from third parties. Based on the results of fault tree analysis (FTA), the root causes of problems or basic events obtained from the results of Fault Tree Analysis (FTA) include corrosive media in the form of water, H2O, O2 and CO2, various causes of soil corrosivity, disturbance of excavation activities, damage due to contact with vehicles/equipment, sabotage and criminal acts in the form of theft. Conclusion, there are 17 basic events that cause pipe leakage (crude oil) at PT Pertamina EP Pendopo Field with 8 minimum cut sets combinations of event occurrence.
