Abstract
Background: Endometriosis is a pathological condition marked with extrauterine growth of the endometrial tissues resulting in a systemic inflammation marked by the increase of serum IL-6 and pain. Ophiocephalus striatus is a species of fish with high protein content, and protein supplementation is known to play a role to reduce inflammation in other conditions. This study aims to measure the changes in inflammatory marker (scored with the serum IL-6 concentration) and pain (scored with VAS) in endometriosis patients. Methods: This study is conducted as a pilot study involving 30 subjects diagnosed with endometriosis (15 subjects receiving only standard treatments and 15 subjects receiving O. striatus extract in addition to the standard treatments). The baseline serum IL-6 concentration and VAS was measured prior to and after eight weeks of treatments. The serum IL-6 concentration measurements were conducted in collaboration with Prodia Clinical Laboratory, while VAS were measured through interview with the subjects. Results: Pre-treatment VAS were 5.00 ± 1.36 in the intervention group and 5.2 ± 1.42 in the control group (p = 0/.697); meanwhile, pre-treatment serum IL-6 concentration in the intervention group were 96.7 (64.5-147) pg/mL and serum IL-6 concentration in the control group were 88.9 (69.2-159) pg/mL (p = 0.917). standard treatments were able to reduce VAS (3.5 ± 1.04 vs. 5.1 ± 1.37; p < 0.001) and serum IL-6 concentration (19.45 [3.1-40.0] pg/mL vs. 92.8 [64.5-156.0] pg/mL; p < 0.001). No significant VAS differences were observed between the intervention and control groups after eight weeks of treatments (3.2 ± 0.77 vs. 3.8 ± 1.21; p = 0.116); further, no significant differences in the VAS reductions were observed between the intervention and control groups (-1.8 ± 1.42 vs. -1.4 ± 1.18; p = 0.410). On the other hand, significant differences in the serum IL-6 concentration were observed after eight weeks of treatments (3.9 [3.1-13.2] pg/mL vs. 28.7 [25.7-40.6] pg/mL; p < 0.001). In addition, the differences of the serum IL-6 concentration reduction themselves were significant (-92.8 [-51.3 to. -143.1] pg/mL vs. -53.7 [-41 to -116.7] pg/mL; p = 0.041). Conclusions: O. striatus extract supplementation did not result in better VAS in comparison with the standard treatments, but O. striatus extract supplementation resulted in significant serum IL-6 reduction in comparison with the standard treatments.
