Abstract
Comparison of the Effectiveness of Phenylephrine 100 mcg and Ephedrine HCl 10 mg Bolus (Impact on Blood Pressure, Heart Rate, Respiration Rate, and Oxygen Saturation) in Spinal Anesthesia for Cesarean Section ABSTRACT Introduction: Hypotension is a common complication in patients undergoing spinal anesthesia for cesarean section. Phenylephrine and ephedrine are two vasopressor agents frequently used to manage this hypotension. This study aims to compare the effectiveness of administering phenylephrine 100 mcg and ephedrine HCl 10 mg bolus in controlling systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), respiration rate (RR), and oxygen saturation (SpO2) in patients undergoing spinal anesthesia for cesarean section. Methods: This study utilized an experimental design with two intervention groups. The values of SBP, DBP, HR, RR, and SpO2 were assessed serially at pre-spinal anesthesia baseline (T0), 5 minutes post-spinal anesthesia (T1), 10 minutes post-spinal anesthesia (T2), 15 minutes post-spinal anesthesia (T3), and 20 minutes post-spinal anesthesia (T4). Data analysis to assess changes and differences in mean hemodynamic parameters was conducted using paired t-test, repeated measures ANOVA, and independent t-test. Results: Analysis using repeated measures ANOVA showed significant results in the SBP variable for both groups (p<0.001; p=0.001), DBP in the ephedrine group (p=0.005), and HR in the ephedrine group (p<0.001). There were no significant results for RR and SpO2 variables (p>0.05). Independent t-test showed significant results (p<0.001) in HR at T1 where the mean HR for ephedrine was higher than that for phenylephrine (79.82 ± 6.71 bpm vs. 97.65 ± 8.33 bpm). Paired t-test showed significant results for SBP T0 vs. T1 in both groups (p<0.001), DBP T0 vs. T1 (p=0.001) and T1 vs. T2 (p=0.024) in the ephedrine group, and HR T0 vs. T1 and HR T1 vs. T2 in the ephedrine group (p<0.001). Conclusion: Comparison of phenylephrine administration is not more effective than ephedrine HCl on systolic and diastolic blood pressure. Both phenylephrine and ephedrine are effective in controlling blood pressure in patients undergoing spinal anesthesia for cesarean section. There is a significant difference in the heart rate variable and no significant difference in the RR and SpO2 variables between the two groups.
