Abstract
Pertusis merupakan penyakit infeksi pernafasan yang disebabkan oleh Bordetella pertussis yang dapat menimbulkan berbagai komplikasi dan kematian. Walaupun demikian, beban penyakit pertusis ini sendiri belum tergambar dengan baik terutama di Negara – Negara Asia Tenggara termasuk di Indonesia. Tujuan penelitian ini untuk menganalisis faktor risiko dan kondisi klinis kasus pertusis di Provinsi Sumatera Selatan. Metode penelitian bersifat kuantitatif dengan desain cross-sectional menggunakan 384 sampel kasus pertusis yang dilaporkan di Provinsi Sumatera Selatan tahun 2019-2023. Hasil analisis univariat menunjukkan bahwa 40,4% kasus pertusis berasal dari kelompok umur < 1 tahun dan menyebabkan kematian (CFR 10,3%) pada kelompok umur tersebut. Kasus yang dilaporkan lebih banyak dilaporkan pada jenis kelamin Perempuan (53,9%) dan 69% kasus belum pernah mendapatkan vaksinasi DPT. Hasil bivariat dengan uji chi square menunjukkan bahwa variabel usia, status imunisasi dan riwayat kontak memiliki hubungan yang signifikan dengan kejadian pertusis dimana pada variabel usia memiliki nilai p value = 0,001 (PR 2,080; 95% CI; 1,363-3,173), variabel status imunisasi memiliki nilai p value = 0,012 (PR 2,084; 95% CI; 1,197-3,629) dan variabel riwayat kontak memiliki nilai p value = 0,000 (PR 4,530; 95% CI; 2,712-7,566). Sedangkan untuk variabel jenis kelamin tidak memiliki hubungan yang signifikan dengan kejadian pertusis dimana nilai p value = 0,765 (95% CI; 0,606-1,390) dan hasil uji multivariat menunjukkan bahwa faktor risiko paling dominan terhadap kejadian pertusis adalah riwayat kontak p value = 0,000 (PR 4,775; 95% CI; 2,841-8,026). Kesimpulan dari penelitian ini bahwa model prediksi faktor risiko dan identifikasi manifestasi klinis berupa batuk paroxysmal, whooping cough, muntah setelah batuk dan apnea mungkin berguna dalam surveilans pertusis sebagai upaya pencegahan dan pengendalian pertusis di masyarakat. However, the burden of pertusis has not been well-documented, especially in Southeast Asian countries, including Indonesia. This study aims to identify the risk factors and clinical conditions associated with pertusis cases in South Sumatra Province. This quantitative study used a cross-sectional design, analyzing 384 pertusis cases reported in South Sumatra Province between 2019 and 2023. Univariate analysis revealed that 40.4% of the pertusis cases occurred in the age group under 1 year, with a case fatality rate (CFR) of 10.3% in this group. Additionally, 53.9% of the cases were female, and 69% of the cases had never received DPT vaccination. Bivariate analysis using the chi-square test showed significant association between the incidence of pertusis and the variables of age, immunization status, and contact history. The age variable had a p-value of p value = 0,001 (PR 2,080; 95% CI; 1,363-3,173), the immunization status variable had a p-value of 0,012 (PR 2,084; 95% CI; 1,197-3,629), and the contact history variable had a p-value of 0,000 (PR 4,530; 95% CI; 2,712-7,566). Meanwhile, the gender variable did not show a significant relationship with the incidence of pertusis, with a p-value of 0.756 (95% CI: 0.606-1.390). Multivariate analysis identified contact history as the most dominant risk factor for pertusis incidence, with a p-value of 0,000 (PR 4,775; 95% CI; 2,841-8,026). The findings of this study suggest that identifying risk factors and clinical manifestations, such as paroxysmal cough, whooping cough, vomiting after coughing, and apnea, could be useful for pertusis surveillance, aiding efforts to prevent and control pertusis in the community.
