Abstract

Introduction: Osteosarcoma is the most common primary high-grade sarcoma of the skeleton. Metastasis in osteosarcoma is the major cause of treatment failure, death, and decreasing survival rate. Recent studies proved that tumor microenvironment is closely related to the pathogenesis of metastasis in osteosarcoma, particularly lymphocyte and macrophage infiltration. The aim of this study is to evaluate the lymphocytes and macrophages density based on clinicopathological features of osteosarcoma in RSUP Dr. Mohammad Hoesin Palembang. Method: A total of 16 Hematoxylin-Eosin (HE) stained slides diagnosed as osteosarcoma were observed under the microscope. The number of infiltrating lymphocytes and macrophages was counted on a 5 high tumor densitiy area then averaged. The median value of these averages was set as a cut-off point to categorized lymphocyte and macrophage density from each sample as a low or high. The data analyzed using cross tabulation and Fisher’s exact test. Results: The majority of osteosarcoma patients are male (68,8%), younger than 40 years of age (87,5%), has a conventional osteosarcoma subtype (68,8%), and has a high number of mitosis (56,3%). Most of the high density lymphocytes and high density macrophages are found in young male patients (<40 years) and in conventional osteosarcoma subtypes. High density lymphocytes was higher in osteosarcoma with low mitosis number. Meanwhile, high density macrophages found significiantly higher in osteosarcoma with high mitosis number (p=0,020). Conclusion: Lymphocyte and macrophage densities are distributed almost equally in all osteosarcoma clinicopathological features exept in the number of mitosis. Higher macrophage density in osteosarcoma is significantly associated with high number of mitosis.
