Abstract
ABSTRACT Factors Affecting the Severity of Illness and Outcomes of Children Confirmed with COVID-19 Coronavirus Disease-19 (COVID-19) is an infectious disease caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus with a significant impact on transmission, morbidity, risk of cross-infection, and mortality. The majority of COVID-19 cases in children are asymptomatic and mild with varying symptoms. Factors from virus and host have a role in SARS-CoV-2 infection. This study aims to determine the factors that influence the severity of illness and treatment outcomes of confirmed COVID-19 children treated at Mohammad Hoesin Hospital Palembang. An analytic observational study with a retrospective cross-sectional method was conducted from January 2020-December 2022. This study obtained 219 pediatric patients aged 1 month to 18 years who were confirmed COVID-19. Risk factors evaluated included age, gender, nutritional status, clinical symptoms or chief complaints, comorbid diseases, laboratory and radiology results, and patient management. Data were analyzed using SPSS version 22, univariate analysis, bivariate analysis with simple logistic regression and chi-squared test, with statistical significance level set at p<0.05. The mortality rate of pediatric patients with confirmed COVID-19 was 12.8% and the majority of patients had asymptomatic/light disease (54.3%). The incidence of COVID-19 based on time period was most prevalent in T1/Alpha variant (43.4%). Comorbidities (PR = 2.771; p = 0.002), anemia (PR = 2.449; p = 0.006), lymphopenia (PR = 2.209; p = 0.034), and oxygen supplementation (PR = 6.676; p = 0.004) had a significant relationship with disease severity. In addition, the use of oxygen supplementation (PR = 37.401; p ≤0.001) had a significant association with COVID-19 treatment outcomes. These findings suggest that comorbidities are the most contributing risk factor to disease severity while the use of oxygen supplementation is the most contributing risk factor to COVID-19 confirmed pediatric care outcomes. .
