Abstract

Background: Cholelithiasis, the formation of gallstones within the gallbladder, affects 6.1% of the global population, with higher prevalence in developed countries. Although it often remains asymptomatic, the condition can lead to serious complications. Laparoscopic cholecystectomy is the preferred treatment, though 2-15% of cases require conversion to open surgery due to various risk factors. Objective: To determine the association between conversion to open surgery and an increased score on the Difficult Laparoscopic Cholecystectomy Scoring System in cholelithiasis patients at Mohammad Hoesin Hospital, Palembang. Methods: This case series, conducted at Mohammad Hoesin Hospital Palembang, involved all patients who underwent laparoscopic cholecystectomy in 2023 and were selected through purposive sampling. Data were analyzed with SPSS version 23, utilizing Spearman correlation and linear regression statistical methods. Results: The case series was conducted in the Digestive Surgery Department of Mohammad Hoesin Hospital Palembang from January to December 2023. Among 30 patients, 86.7% had low scores, with only one patient requiring conversion. The factors most influencing the difficulty of laparoscopic cholecystectomy leading to conversion were sex, history of cholecystitis, and pericholecystic fluid. Spearman's analysis revealed no significant correlation between the scoring system and conversion (r = -0.318, p = 0.087). Conclusion: The association between the incidence of conversion to open surgery and an increased score on the Difficult Laparoscopic Cholecystectomy Scoring System was weak and negative, with no statistical significance. Further studies with larger sample sizes and balanced independent variable distribution are recommended to improve reliability and minimize bias.
