Abstract

Introduction: Blood transfusions are required in the management of postpartum hemorrhage, which is the leading cause of maternal death. The incidence of transfusion in patients with postpartum hemorrhage has increased over the years. Before a patient is transfused, each blood request is crossmatched. If the blood is crossmatched but not transfused, the patient still has to pay for the crossmatch fee and the bag as a service charge. This indicates a waste of reagents, increased financial burden and increased staff workload. The efficiency of blood use can be evaluated by the Crossmatch to Transfusion Ratio (CTR) indicator. This study aims to determine the description of Crossmatch to Transfusion Ratio in postpartum hemorrhage patients at Dr Mohammad Hoesin Hospital Palembang. Method: This study is descriptive observational with cross-sectional design, using secondary data, from SIDORA application and medical records of postpartum hemorrhage patients who need blood transfusion at Dr. Mohammad Hoesin Palembang Hospital for the period of May 2022 - June 2024 which meet the inclusion criteria and don’t meet the exclusion criteria. Data were analyzed using Microsoft Excel and SPSS version 29. Results: Postpartum hemorrhage patients were mostly in the age group of 20 - 35 years (59,8%), had no history of disease during pregnancy (76,4%), parity status 2 - 3 (51,4%), pretransfusion Hb level <7 (52,8%), and posttransfusion 7 - 9.9 g/dL (54,2%). The number of blood bag requests that were crossmatched was 352 blood bags. The number of blood bags transfused was 325 blood bags. The overall CTR value of blood bags was 1.08. The CTR values for WB and PRC blood components were 1 and 1.08. Compatible and incompatible crossmatch test results on crossmatched blood bags were 344 blood bags (97.7%) and 8 blood bags (2.3%). Conclusion: The CTR value in postpartum hemorrhage patients at Dr. Mohammad Hoesin Palembang Hospital for the period May 2022 - June 2024 was 1.08.
