Abstract

Background: Epicanthus is a fold of skin at the nasal bridge and upper eyelid that covers the medial canthus. This condition is common in Asian populations (40–90%) but rare in non-Asians (2–5%). Epicanthus may represent a normal anatomical variation or be associated with syndromes such as Down syndrome, Turner syndrome, or blepharophimosis syndrome. Types of epicanthus include tarsal, palpebral, supraciliary, and inversus, with variations ranging from mild to severe. In some cases, epicanthus causes aesthetic or clinical issues, such as pseudostrabismus, pseudoepiblepharon, entropion, and trichiasis. Most cases do not require surgery, but correction through epicanthoplasty or blepharoplasty can be performed if necessary. This study aims to identify the clinical characteristics of epicanthus in elementary school children in Palembang in 2024. Methods: This study is a descriptive observational with a cross-sectional design using questionnaires and direct observation of elementary school children at SD Negeri 32, SD Negeri 18, SD IT Royal Islamic, and SD Xaverius 01 in Palembang from July to October 2024. Participants met inclusion criteria and did not fall under exclusion criteria. Data was processed using Microsoft Excel and SPSS. Results: The prevalence of epicanthus among elementary school children in Palembang in 2024 was 102 out of 157 samples (65%). It was most common in females (51%) and predominantly found in the Palembang Malay ethnicity (50%). Only 1% exhibited pseudostrabismus. No cases of pseudoepiblepharon, entropion, or trichiasis were identified. The most common type was tarsal epicanthus (88.2%), the most frequent degree was mild epicanthus (71.6%), and it was predominantly bilateral (79.4%). Conclusion: The clinical characteristics of epicanthus identified include pseudostrabismus and epiblepharon, with the most common type being tarsal epicanthus, predominantly mild in degree, and bilateral in laterality.
