Abstract

Background: The tear film plays a very important role in optical quality because it is the most anterior, thin, and uniform refractive surface. Astigmatism is a refractive disorder in which a focal point does not form because the refracted light falls on different meridians. Regular astigmatism can be affected by the tear film, causing additional aberrations in the optical system that can impact visual quality. Objective: To determine the effects of the tear film on the quality of vision in patients with regular astigmatism. Methods: This research is an observational study with a cross-sectional design, conducted at the eye clinic of the refraction division of Mohammad Hoesin General Hospital Palembang and the South Sumatra Provincial Eye Hospital from May to September 2023, involving 170 eyes from 85 research samples. Patient data with astigmatism were recorded based on the results of the Nidek® auto refractometer. In patients, anamnesis was conducted using the Ocular Surface Disease Index (OSDI) and Quality of Vision (QoV) questionnaires. Measurements of tear breakup time (TBUT), Schirmer I, and contrast sensitivity using the Pelli-Robson chart were conducted. Statistical data analysis was conducted using SPSS version 27. Results: There is a significant relationship between the degree of tear film instability (TFI) and the degree of astigmatism (p-value = 0.000), and tear film instability (TFI) can affect the quality of vision (p-value = 0.000). Conclusion: Higher degrees of astigmatism are associated with increased tear film instability (TFI). Patients with higher astigmatism tend to have higher Quality of Vision (QoV) scores, suggesting that tear film instability can worsen visual quality in patients with regular astigmatism.
