Abstract
Indonesia ranks second as the country with the highest tuberculosis (TB) cases in the world. TB disease is caused by the acid-resistant bacillus Mycobacterium tuberculosis (Mtb), which generally infects the lungs. The level of susceptibility to Mtb infection is influenced by host immune response factors, one of which is through the response of pro-inflammatory cytokine IL-18. Single nucleotide polymorphism (SNP) rs1946518 C/A in the IL-18 gene promoter is a polymorphism caused by the substitution of cytosine to adenine at position 607 (- 607C/A), resulting in a decrease in the efficiency of the IL-18 gene in inducing cytokine expression. The SNP may increase the susceptibility of Mtb infection to develop into active pulmonary TB in the host body. This study aims to identify the description and frequency distribution of IL-18 -607C/A gene SNPs in sputum isolates of active pulmonary TB patients in Palembang through the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). This laboratory observation study used 31 sputum isolates of pulmonary TB patients from DNA collections obtained from the Microbiology Section, Faculty of Medicine, Sriwijaya University. Identification procedures were conducted at the Biotechnology Laboratory, Faculty of Medicine, Sriwijaya University in October- December 2024. This study only obtained two samples from 31 sputum isolates of active pulmonary TB patients in Palembang City, showing an IL-18 gene DNA band. However, non-specific DNA bands were also found in both samples, so those samples did not meet the requirements for continuing the RFLP process. In conclusion, this study only performed the procedure until PCR visualization with the results of 6.4% of the total positive samples for the IL-18 gene marked by a DNA band of 171bp without identifying the IL-18 promoter SNP.
