Abstract

Background: Cardiomegaly refers to an abnormal enlargement of the heart, which can be detected through various diagnostic methods, with echocardiography regarded as the gold standard. However, in patients who are bedridden, along with the limitations in healthcare infrastructure at some medical facilities, antero-posterior (AP) chest radiography serves as a more accessible alternative. This study aims to evaluate the accuracy of AP chest radiography in detecting cardiomegaly at RSUP Dr. Mohammad Hoesin. Method: A descriptive observational diagnostic study with a cross-sectional design, involving a sample of 68 inpatients at RSUP Dr. Mohammad Hoesin. Results: The predominant characteristics of cardiomegaly patients identified in this study were male (n= 43, 63,2%), aged 55–64 years old (n=17, 25%). Receiver Operating Characteristic (ROC) analysis revealed that the optimal cut-off value of the Cardiothoracic Ratio (CTR) for detecting cardiomegaly was >55%, with an Area Under Curve (AUC) of 0.743 (95% CI, 0.622–0.841). The diagnostic performance of AP chest radiography demonstrated a sensitivity of 81.13% (95% CI, 68,028%–90,563%), specificity of 60% 60% (95% CI, 32,287%–83,664%), positive predictive value of 87.75% (95% CI, 79,186%–93,104%), negative predictive value of 47.37% (95% CI, 31,368%–64,318%), positive likelihood ratio of 2.028 (95% CI, 1,077–3.821), negative likelihood ratio of 0.314 (95% CI, 0,157–0,630), and an overall accuracy of 76.47% (95% CI, 64,621%–85,912%). Conclusion: Based on the results, AP chest radiography demonstrates potential as a preliminary screening tool for detecting cardiomegaly, though it is not sufficiently robust as the primary diagnostic method.
