Abstract

Background: Crossmatch to Transfusion Ratio (CTR) is one of the indicators to assess the efficiency of blood utilization. Blood transfusion is indicated to be efficient if the CTR value is ≤ 2,5. Inefficient blood utilization leads to reduced blood shelf life, decreased blood stock, waste of blood, materials, and resources, and increases the cost of services that must be paid by patients. Orthopedics is one of the departments with high transfusion demand because orthopedic procedures are associated with large amounts of bleeding. This study aims to determine the description of CTR in orthopedic patients at Dr. Mohammad Hoesin Hospital Palembang. Method: This study is a descriptive observational study with a cross sectional approach. The data used were secondary data in the form of medical records and Sidora application data from orthopedic patients at Dr. Mohammad Hoesin Hospital for the period January 2023 - December 2023 who met the inclusion criteria and did not meet the exclusion criteria. Data were processed using Microsoft Excel 2021 and SPSS V27. Result: The CTR value for orthopedic patients at Dr. Mohammad Hoesin Hospital was 1,79, indicating efficient blood transfusion. The number of blood units that were cross-matched was 589 units, and 329 of them (55,9%) were transfused to patients. All blood components that were cross-matched were PRC (100%), with compatible (98,2%) and incompatible (1,2%) results. The median age of orthopedic patients was 52 years, predominantly female (57,5%), with a mean preoperative Hb level of 11,7 g/dL and the most common procedure requiring blood transfusion was hip arthroplasty (26,2%). Conclusion: The CTR value of orthopedic patients at Dr. Mohammad Hoesin Hospital is 1,79, which indicates the implementation of efficient blood transfusion.
