Abstract

Colorectal cancer is a malignancy of the large intestine, caused 1.9 million cases and 900,000 deaths in 2020. Chemotherapy, the primary treatment for advanced-stage cancer, can lead to significant comorbidities, including anemia, Chemotherapy-Induced Anemia (CIA) occurs due to inflammation that disrupts erythropoiesis and bone marrow toxicity, negatively affecting quality of life and treatment effectiveness. This study aims to determine the prevalence of chemotherapy-induced anemia in colorectal cancer patients at Dr. Mohammad Hoesin General Hospital, Palembang, in 2023. This study is an analytical observational research with a cross-sectional design, used secondary data from medical records collected during 2023. Variables include age, gender, education level, cancer stage, cancer location, colostomy, chemotherapy regimen, CEA levels, and hemoglobin levels. Data were analyzed using univariate and bivariate methods (chi-square and paired T-test) with SPSS version 27. Among 134 patients, the prevalence of anemia was 77.61%. Average hemoglobin levels decreased from 11.54 g/dL before chemotherapy to 10.8 g/dL after, with a significant decline between the third and fourth cycles (p = 0.019). Grade 1 anemia was most common, peaking at the fifth cycle (66.7%). Anemia was more frequent in males (41.8%), patients aged 18–59 years (53.7%), those with stage IV cancer (47.2%), undergoing FOLFOX therapy (39.6%), with rectal cancer (44%), and colostomy users (52.7%). Most patients experienced mild anemia, predominantly in males, aged 18–59 years, with stage IV cancer, using FOLFOX regimens, rectal cancer location, and colostomy users. Chemotherapy has a cumulative impact on hematopoiesis. Therefore, effective anemia management is essential to improve treatment outcomes and patient’s quality of life.
