Abstract

Background : Caesarean section delivery has been significantly rising over the past two decades, including in Indonesia, where the rate has reached 17.6%, surpassing the World Health Organization's recommended range (10-15%), and is even higher in certain regions such as Jakarta. This increase raises concerns as C-section performed without clear medical indications may lead to serious complications. Therefore, vaginal delivery in women with a previous C-section are crucially considerated. This study aims to analyze the factors influencing delivery outcomes in pregnant women with a history of C-section at RSUP Dr. Mohammad Hoesin Palembang from 2020 to 2023. Method : An observational analytical method with cross-sectional design was conducted, by utilizing from patient medical records at RSUP Dr. Mohammad Hoesin Palembang 2020‒2023. The variables studied include maternal age, BMI, birth weight, gestational age, history of vaginal delivery, inter-delivery interval, previous C-section indications, number of previous C-section, cervical dilation, and fetal presentation. Results : Previous vaginal delivery and cervical dilation significantly associated to influencing delivery in women with prior C-section. A history of vaginal delivery increases the likelihood of successful vaginal birth up to nine times, while cervical dilation ≥4 cm enhances by 5.4 times. Conclusion : There is a significant association between maternal BMI (p=0.022), history of vaginal delivery (p<0.001), number of previous C-section (p<0.001), and cervical dilation (p<0.001) with delivery outcomes in pregnant women with a C-section history, with a notable impact on the history of vaginal delivery.
