Abstract

Background: Sleep is a fundamental biological function that plays an important role in body recovery, including supporting cognitive function and brain metabolism. Sleep disorders such as insomnia are common in ischemic stroke patients and can have a negative impact on the post-stroke recovery process. Aim: Knowing the factors that influence insomnia in ischemic stroke patients. Method: This study is a cross-sectional observational analytic with primary data from questionnaire interviews and secondary data from medical records. Subjects were 105 ischemic stroke patients at the Neurology Polyclinic of Dr. Mohammad Hoesin Hospital Palembang in May-July 2024. Data collected included sociodemographics, stroke clinical characteristics, comorbid diseases, depression, anxiety, pain, insomnia, medications, and sleep hygiene for further analysis. Results: 42.9% of ischemic stroke patients experienced insomnia, with mild insomnia accounting for 26.7%, moderate insomnia for 13.3%, and severe insomnia for 2.9%. Obstructive sleep apnea (OR: 22.718), sleep hygiene index (OR: 6.490), and low education level (OR: 3.453) are determinants related to insomnia in ischemic stroke patients. Indirect factors associated with insomnia in ischemic stroke patients include depression, the number of comorbid diseases, pain, diabetes mellitus, a history of insomnia, anxiety, and the onset of stroke. Conclusion: There is a complex relationship between various physical, psychological, and social risk factors for the incidence of insomnia after ischemic stroke.
