Abstract

Background: Carpal Tunnel Syndrome (CTS) is the most common neuropathy affecting the upper extremities. Nerve Conduction Study (NCS) helps differentiate CTS from other conditions, but it is invasive, expensive, and only available at certain healthcare centers. Ultrasonography (USG) is more comfortable, cheaper, non-invasive, and can assess the anatomical parameters of the median nerve. The Cross-Sectional Area (CSA) is one of the parameters evaluated and is considered accurate for diagnosing CTS, although the exact CSA cutoff value remains controversial, with thresholds varying between 9.15 mm² and 10.7 mm². Objective: To assess the accuracy of ultrasonography (USG) examination compared to nerve conduction study (NCS) in patients with carpal tunnel syndrome (CTS) at Mohammad Hoesin Hospital, Palembang Methods: Diagnostic test to evaluate various CSA measurement variations on USG, including CSAc, CSAp, mean CSAcp, and ∆CSA, compared with NCS. Results: CSAc showed a sensitivity of 89.6%, specificity of 77.8%, with a cutoff point ≥10.7 mm², and an accuracy of 88.3%. CSAp had a sensitivity of 72.7%, specificity of 66.7%, with a cutoff point ≥9.0 mm², and an accuracy of 72.1%. Mean CSAcp showed a sensitivity of 67.5%, specificity of 66.7%, with a cutoff point ≥10.2 mm², and an accuracy of 67.4%. ∆CSA exhibited a sensitivity of 92.2%, specificity of 88.9%, with a cutoff point of 2.0 mm², and an accuracy of 91.9%. Conclusion: Measurement of ∆CSA has an advantage over other measurement parameters. The sensitivity of ∆CSA is almost identical, but its specificity is lower than that of NCS. .
