Abstract
Sepsis is a life-threatening condition that occurs when the body’s immune system fails to respond properly to an infection, leading to organ dysfunction. It remains a significant global health issue, particularly in low- and middle-income countries. This study aimed to examine the distribution of bacterial types and their antibiotic sensitivity patterns in blood cultures from sepsis patients. Using a retrospective descriptive design, we analyzed blood culture data from sepsis patients treated in the ICU at RSMH Palembang. A total of 14 samples were included in this study. The majority of sepsis patients were adults or pre-elderly, with an equal distribution of men and women. Diabetes was the most common comorbidity, seen in 42.9% of cases, and the median length of stay in the hospital was 5.5 days. Among the Gram-positive bacteria, Coagulase-negative Staphylococci (CoNS) were the most frequently identified, while Escherichia coli, Klebsiella pneumoniae ssp pneumoniae, and Acinetobacter baumannii were the dominant Gram-negative bacteria. Linezolid, nitrofurantoin, and vancomycin were the most effective antibiotics against gram-positive bacteria, while amikacin, ertapenem, gentamicin, meropenem, nitrofurantoin, piperacillin-tazobactam, and sulfamethoxazole-trimethoprim showed good sensitivity against gram-negative bacteria. Interestingly, tigecycline was found to be effective against both gram-positive and gram-negative bacteria. Penicillin-class antibiotics displayed the highest resistance against Gram-positive bacteria. Additionally, this study identified Extended-Spectrum-Beta-Lactamase (ESBL) producing bacteria and Methicillin-Resistant Staphylococcus aureus (MRSA) as multidrug-resistant organisms (MDROs). In conclusion, Coagulase-negative Staphylococci (CoNS) and Escherichia coli were the most common bacteria causing sepsis in this sample. Linezolid, nitrofurantoin, and vancomycin were the most effective antibiotics for Gram-positive infections.
