Abstract

Diffuse large B cell lymphoma (DLBCL) is a subtype of non-Hodgkin lymphoma (NHL) characterized by large B lymphoid cells, where the nucleus exceeds the size of a normal lymphocyte nucleus, exhibiting a spectrum of cellular progenitors and consistent nuclear morphology. Platelets and lymphocytes are commonly assessed to predict the prognosis of DLBCL patients. The platelet-to-lymphocyte ratio (PLR) has emerged as a novel prognostic marker, as this ratio represents the combined concept of platelet activity and inflammatory pathways, both of which play a role in the pathogenesis of malignancy. The aim of this study is to examine the correlation between the pre-treatment platelet-to-lymphocyte ratio and the prognosis of DLBCL patients. This study is an analytical observational with a retrospective cohort design, accompanied by survival analysis. The sample for this study consists of DLBCL patients from Dr. Mohammad Hoesin General Hospital Palembang in 2020–2023, who meet the inclusion criteria and were selected using consecutive sampling technique. The data will be analyzed using ROC curve, Kaplan-Meier curve, Mantel-Cox test, Cox regression, and time-independent Cox regression. The ROC curve analysis determined cut-off point of platelet-lymphocyte ratio was 193,1 with 70,4% in sensitivity and 69,2% in specificity. Based on the Kaplan-Meier curve analysis yielded a median overall survival was 0.58 years and the variables that met the proportional hazards assumption were stage and platelet-to-lymphocyte ratio. The results of the bivariate analysis showed a p-value for stage was <0.001 and a p-value for the platelet-to-lymphocyte ratio was 0.022. The results of the multivariate analysis showed a p-value for stage was <0.001 and a p-value for the platelet-to-lymphocyte ratio was 0.155. Stage and platelet-to-lymphocyte ratio are significantly correlated with overall survival with stage being the most strongly correlated factor.
