Abstract

Background: Hepatocelluler carcinoma (HCC) is a malignant tumor originating from hepatocytes and is the most common primary liver tumor. In determining the stage of HCC, it is essensial to assess the size and number of tumor lesions, liver function, physical performance, tumor invasion, extrahepatic metastasis, and clinical symptoms. Several biomarkers such as neutrophil lymphocyte ratio (NLR), alpha fetoprotein (AFP), and protein induced by the absence of vitamin k or antagonist-II (PIVKA-II) can be used as diagnostic and prognostic markers for HCC. This study aims to analyze the correlation between NLR, AFP, and PIVKA-II with the stages of HCC. Methods: A observational study was conducted to analyze the correlation between NLR, AFP, and PIVKA-II with the stage of HCC patient stage at the Department of Internal Medicine, Dr. Mohammad Hoesin Hospital, Palembang, from April to July 2024. The HCC staging was classified based on the Barcelona clinic Liver cancer (BCLC) system. NLR, AFP, and PIVKA-II were assessed using ELISA and spectrophotometry. All data were analyzed using SPSS version 26. Results: A total of 38 HCC patients who met the inclusion criteria were included in the study. There was a weak positive correlation between NLR and HCC stage (r = 0.334; p = 0.040), a moderate positive correlation between AFP and HCC stage (r = 0.499; p = 0.001), and a strong positive correlation between PIVKA-II and HCC stage (r = 0.797; p = 0.000). NLR had no effect on HCC stage, while PIVKA-II and AFP were found to have a significant sequential effect on BCLC stage. Conclusion: There is a significant positive correlation between NLR, AFP and PIVKA-II with HCC stage. PIVKA-II has the highest strength compared to NLR and AFP.
