Abstract

Hypertension home care is home care carried out on hypertension patients who have lost control so that they visit again to the health center and receive treatment with the aim of improving their quality of life. The purpose of this study was to determine description the characteristics of hypertension patients undergoing home care at the Dr. Soetomo Health Center work area. The study method used quantitative descriptive. The samples were 84 respondents. This study was conducted by analyzing secondary data on home care patients through the use of internship data and campus independent studies with the competency outcomes of monitoring and evaluating home care programs. The results of the study obtained the characteristics of the majority of respondents were >60 years old (58.3%), respondents were female (81.0%), half of the respondents had a high school/equivalent education level (50.0%), respondents were unemployed (72.6%). Most of respondents were visited (73.97%) in home care 1, 2 and 3. The average (15.4%) consumption of antihypertensive drugs showed an increase from home care 1 to 3. The average (14.3%) of respondents' complaints decreased from home care 1 to 3. Most of respondents had suffered from hypertension since <1 year (56.0%). Most of respondents had a history of comorbidities (52.4%). Visits of hypertension patients to the health center increased (39.05%) in the home care program. Home care can be costumized to the needs hypertension patients were male, young, low-educated, and working hypertension patients through a flexible home care approach and equipped with simple but effective education in order to be able increase patient participation and compliance with hypertension management.
