Abstract

Background: Mutations in the rpoB gene, which encodes the β-RNA-polymerase subunit, occur in an 81-bp area called the Rifampicin Resistant Determining Region (RRDR) in codons 507 to 533. This point mutation area is used to rapidly identify rifampicin-resistant isolates. Mutations in the rpoB gene area in MTb, in addition to having changes in phenotypic properties that cause resistance to the mechanism of action of the drug rifampicin. It turns out that there are also changes in the phenotypic properties of the MTb cell wall structure. The decrease in sulfoglycolipid structure, the increase in LAM structure, and the thicker mycolic acid layer make rpoB mutant MTb have an increased ability to avoid the human immune system. Thus, MTb is able to survive and proliferate, which can lead to higher virulence levels. This change in phenotypic properties has been carried out by several studies in vitro. However, no published studies have assessed the relationship clinically. Method: This study was a cross-sectional analytical observational study. Using a total sampling technique, all patients in the study population who met the inclusion and exclusion criteria during the period 2014-2023 at Dr Moh Hoesin Hospital Palembang were taken. Data were analysed bivariately and multivariately using SPSS. Result: 315 samples were collected through medical records and SITB data. There was a significant association between the mutation area of specific rpoB gene and sputum BTA index in the mutation area of probe D (p=0.003, PR 1.6) and probe E (p=0.001, PR 0.6). There was a significant association between specific rpoB gene mutation area and ct index in mutation area of probe D (p=0.001, PR 1.4) and probe E (p=0.001, PR 0.75). There was a significant association between specific rpoB gene mutation area and chest X-ray lesion area in the mutation area of probe B (p=0.039, PR 1.6). Conclusion: There is a relationship between certain MTb rpoB gene mutation areas with sputum bacterial load and chest X-ray lesion width area in drug-resistant pulmonary TB patients at RSMH Palembang. The rpoB gene mutation areas are probe areas D and E with high BTA index and high ct index. And, probe area B on chest X-ray lesion area.
