Abstract

Lock Out Tag Out (LOTO) is a safety procedure to protect the worker around machine or equipment during service or maintenance activity. This research was conducted at PT Pupuk Sriwidjaja Palembang because only tag out devices are oftenly used. This research aimed to analyze the implementation of LOTO as a control of hazardous energy in PT Pupuk Sriwidjaja Palembang. This research was qualitative with descriptive approach. The informants consisted of two main informants and nine ordinary informants. Determination of informants as information sources used purposive sampling method.the compiling data method was by indepth interview and observation. The result of this reseacrh showed that the implementation of LOTO as a control of hazardous energy has been quite well implemted in Pabrik Pusri IIB except in Pabrik Pusri III, IV, and IB. The energy controlled by isolating the breaker or valve that performed by the work supervisor and operator. The energy control was not fully compatible with LOTO procedures that regulated in SMK3 document about Work Instruction of LOTO No. 3 SMK3 039. LOTO devices available at PT Pupuk Sriwidjaja Palembang such as pad lock, tag and chain are available in small quantities, not evenly distributed and not fully meet the standards. The implementation of LOTO socialization not fully delivered to all workers so that caused lack of knowledge and implementation of LOTOin the field and the evaluation of LOTO do not have a fixed schedule in a year. Based on the result can be concluded that application of LOTO in PT Pupuk Sriwidjaja Palembang not fully implemented and not completely followed the procedures. so it is suggested that the company increase the supervision on the implementation of LOTO in the field, add LOTO facility and distributed it equally, doing the socialization, inspection and training of LOTO for authorized worker, and doing LOTO evaluation every year to improve worker understanding and optimizing implementation of LOTO in the field.
