Abstract

Background: One of the most debilitating non-motor symptoms of Parkinson's disease is orthostatic hypotension, characterized by a decrease of at least 20 mmHg in systolic blood pressure or 10 mmHg in diastolic blood pressure within 3 minutes of standing or tilting the head by 60 degrees. Orthostatic hypotension in Parkinson's disease has a relatively high prevalence (9.6%−64.9%), however is often overlooked by clinicians. It is believed that orthostatic hypotension in Parkinson's disease can accelerate cognitive impairment. Aim: To identify the association between orthostatic hypotension with cognitive impairment in Parkinson's disease patients at a neurology outpatient clinic. Methods: This was an analytic observational study with a cross-sectional design using primary data from interviews based on checklists, questionnaires, and blood pressure examinations. The study subjects consisted of 33 Parkinson's disease patients with orthostatic hypotension and 34 Parkinson's disease patients without orthostatic hypotension. Results: Multivariate analysis revealed a significant relationship between orthostatic hypotension and cognitive impairment in Parkinson's disease patients (p=0.003, OR=6.33), although the confounding factor of age could not be excluded. A significant difference in the attention domain was observed in the orthostatic hypotension group (p=0.045, Z=-2.001). Conclusion: There is a significant relationship between orthostatic hypotension and cognitive impairment in Parkinson's disease patients, particularly in the attention domain.
