Abstract

Introduction: Head injury is a significant cause of morbidity and mortality worldwide. A scoring system plays an important role in predicting mortality rates. This study aims to determine the accuracy of the Rotterdam score in predicting mortality in head injury patients undergoing surgery. Methods: This study is a retrospective study using quantitative analysis. All head injury patients undergoing surgery at Dr. Mohammad Hoesin Hospital, Palembang were included in the study and analyzed using the Rotterdam score. Univariate, bivariate, and multivariate analyses were performed using SPSS v25 software. Results: A total of 56 head injury patients were obtained in this study. Most of the patients were male, 37 (66.1%). The most common age group found was the 18-59 year age group, which was 24 (42.9%). Mild head injury was the most GCS found which was 24 (42.9%). Mortality in this study was 10 people (17.8%) and dominated by men as many as 6 (60%) and the 18-59 age group was 4 people (40%). On the ROC curve, a cut-off of 4.5 was obtained with an Area Under Curve (AUC) value of 0.953. The Rotterdam score has a fairly accurate accuracy value with a sensitivity of 80%, specificity of 97.8%, PPV of 88.8%, and NPV of 95.7%. In the multivariate analysis, the classification of head injury was found to have the most significant on patient mortality, with a p-value of 0.005. Conclusion: The Rotterdam score is very accurate in predicting mortality in head injury patients undergoing surgery. However, research with a larger sample is needed to confirm this study's results.
