Abstract
Background: Chronic hepatitis B infection progresses through distinct phases that reflect the dynamic interplay between the hepatitis B virus (HBV) and the host immune system. Exhausted CD8+ T cells are marked by increased PD-1 expression, while soluble PD-1 (sPD-1), a soluble form of the PD-1 receptor, regulates immune responses. Both sPD-1 and CD8+ T cells play critical roles in determining infection phases and characterizing the progression of chronic hepatitis B. This study aims to investigate the relationship between sPD-1 levels and CD8+ T cell counts across different phases of chronic hepatitis B infection. Method: This analytical observational study with a comparison was conducted at Mohammad Hoesin General Hospital, Palembang, from June to November 2024, involving chronic hepatitis B patients who had not received antiviral therapy. sPD-1 levels were measured using the ELISA method, and CD8+ T cell counts were analyzed by flow cytometry. Data were processed using SPSS version 26. Results: Sixty chronic hepatitis B patients met the inclusion criteria. The reactivation phase showed the highest sPD-1 levels (median: 351.8 pg/mL; range: 196–743.4 pg/mL), while the carrier inactive phase had the lowest levels (mean: 214.78 ± 36.05 pg/mL). sPD-1 levels were significantly higher in the reactivation phase compared to the immune tolerant and carrier inactive phase. CD8+ T cell counts were significantly lower (p<0.05) in the immune active and reactivation phases compared to the immune tolerant phase. Age ≥ 45 years and high sPD-1 levels in HBeAg-positive patients were significant independent predictors of immune active phase, while high sPD-1 levels were the only predictor of the reactivation phase in HBeAg-negative patients. sPD-1 has the potential role as biomarker for therapeutic indications in chronic hepatitis B infection. Conclusion: There is a relationship between sPD-1 levels and CD8+ T cell counts with chronic hepatitis B infection phase in antiviral naive patient.
