Abstract
Background: Uncontrolled type 2 diabetes mellitus (T2DM) is a major health problem characterized by chronic hyperglycemia and systemic inflammation, where Interleukin-1β (IL-1β) plays a key role in worsening insulin resistance. Diacerein, an IL-1β inhibitor, has shown potential to reduce inflammation and improve glycemic control. This study aimed to assess the efficacy of diacerein on IL-1β levels and glycemic control in uncontrolled T2DM patients. Methods: This double-blind, randomized clinical trial was conducted between July and October 2024 at the Endocrine Metabolic Diabetes Outpatient Clinic of Mohammad Hoesin General Hospital. A total of 34 patients with uncontrolled T2DM were randomized into diacerein (n=17) and placebo (n=17) groups using block-permuted randomization. Patients received either diacerein 50 mg or placebo twice daily for 90 days. Baseline and post-intervention assessments included serum IL-1β, glycated hemoglobin (HbA1c), and fasting blood glucose (FBG). Results: Of the 34 patients enrolled, 4 (2 in each group) dropped out, leaving 30 patients (15 per group) for the final analysis. Baseline characteristics were similar between groups (p>0.05). After 90 days, the diacerein group exhibited a significant reduction in HbA1c (-0.7%; p=0.002) and FBG (-66.47 mg/dL; p=0.004) compared to the placebo group, which showed no significant changes. IL-1β levels demonstrated minimal changes in both groups, with no statistical significance (p>0.05). Multivariate analysis confirmed diacerein as an independent predictor of HbA1c and FBG reduction (p=0.025 and p=0.019, respectively). No severe adverse effects were observed, with only mild gastrointestinal symptoms reported in the diacerein group. Conclusion: The addition of diacerein significantly improved glycemic control (HbA1c and FBG) in uncontrolled T2DM patients, without significant effects on IL-1β levels.
