Abstract
Background: Fluid resuscitation is vital in sepsis management, but excessive accumulation can lead to fluid overload (FO), potentially worsening patient outcomes. This study aimed to evaluate the association between fluid overload and key clinical outcomes: ICU length of stay, ventilator duration and mortality. Methods: A retrospective observational study was conducted on 260 adult sepsis patients admitted to the ICU between January 2022 and December 2024. Fluid overload was categorized by using ROC curve. Statistical analyses included chi-square, t-tests, ROC curves, and multivariate logistic regression to evaluate the effect of fluid overload to ICU length of stay, ventilator duration and mortality. Results: Fluid overload was significantly associated with increased mortality, prolonged ventilator use, and extended ICU stay (p < 0,05). ROC curves demonstrated good predictive value of fluid balance on ICU length of stay, ventilator duration and mortality. Multivariate analysis confirmed fluid overload as an independent risk factor for all clinical outcomes after adjusting for confounders. Discussion: The findings support previous evidence that fluid overload contributes to organ dysfunction and poor prognosis in sepsis. Pathophysiologically, fluid accumulation may lead to pulmonary edema, renal congestion, and impaired oxygenation. These complications likely explain the prolonged ICU length of stay, ventilator duration and mortality. Fluid management beyond the initial resuscitation phase must be guided by clinical assessment and close monitoring of cumulative fluid balance. Conclusion: Fluid overload is a strong predictor of adverse outcomes in sepsis. Individualized and conservative fluid strategies are critical to improving survival and reducing complications in ICU sepsis patients.
