Abstract

Background: The maternal mortality rate in Indonesia is one of the highest in Southeast Asian countries, with one of the most common causes is infection caused by PPROM. PPROM refers to the breaking of the amniotic sac before the onset of labour, before 37 weeks of pregnancy, it is occurs in approximately 8–10% of pregnancies, associated with 18–20% of preterm deliveries, and perinatal mortality. The commonly used nitrazine test, has limitations due to false positives caused by blood, semen, or infection. IGFBP-1 offers a promising alternative diagnostic marker due to its higher specificity. However, data on its diagnostic performance and clinical utility in PPROM cases remain limited. Therefore, this study aims to compare the accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of IGFBP-1 testing with the nitrazine test, and to assess maternal and neonatal outcomes in PPROM-affected pregnancies. Method: This study is a accuracy test study with research samples third trimester pregnant women with PPROM who came to Moh. Hoesin Palembang Hospital from February to September 2024. IGFBP-1 was evaluated using vaginal samples with the immunoassay method. Result: In this study, the results showed that IGFBP-1 examination to detect PPROM in pregnancy has very good examination accuracy (0.889) with a sensitivity of 97.2% and a specificity of 80.6% compared to nitrazine examination with a moderate accuracy value (0.542), sensitivity of 58.3%, and specificity of 50%, so that IGFBP-1 examination can be used as an initial examination of PPROM at Moh. Hoesin Hospital, Palembang. This study also showed that preterm PROM increases the risk of puerperal fever by threefold and neonatal respiratory disorders by sixfold. Conclusion: IGFBP-1 examination in detecting premature rupture of membranes in preterm pregnancy has very good examination accuracy with high sensitivity and specificity compared to nitrazine examination, so that IGFBP-1 examination can be used as an initial examination for premature rupture of membranes.
