Abstract

Background: Endometriosis is a common gynecological disease that frequently causes chronic pelvic pain and infertility. Definitive diagnosis often requires invasive surgical procedures. Therefore, non-invasive markers such as anogenital distance (AGD), ultrasonography (USG), and CA-125 levels are needed to support diagnostic accuracy.. Methods: This case-control study included 56 reproductive-age women presenting to the Obstetrics and Gynecology Outpatient Clinic at Dr. Moh. Hoesin General Hospital Palembang between November 2024 and April 2025. Subjects were divided into endometriosis (n=28) and non-endometriosis (n=28) groups. AGD was measured using a digital caliper, USG was performed by an experienced sonographer, and serum CA-125 was measured using an immunoassay method. Diagnosis was confirmed via histopathology results. Results: Mean AGD-AC and AGD-AF measurements showed no significant difference between groups (AGD-AC: 65.65±8.42 mm vs. 66.42±15.88 mm; AGD-AF: 26.27±7.65 mm vs. 26.99±7.68 mm; p>0.05). USG demonstrated a sensitivity of 78.6% and specificity of 92.9%. CA-125 levels were significantly higher in the endometriosis group (160.67±243.28 IU/mL; p=0.04) with a sensitivity of 89.3% and specificity of 82.1% at a cut-off value of 30.7 ng/mL. Conclusion: USG is effective in detecting endometriosis, and CA-125 may serve as a complementary biomarker. AGD was not shown to be a significant diagnostic marker in this study.
