Abstract

Abstract Introduction Kidney fibrosis is a key pathological feature of chronic kidney disease (CKD) and plays an important role in the progression of CKD to end-stage renal disease (ESRD). Unilateral ureteral obstruction (UUO) is a widely used model to study obstructive nephropathy. In the UUO model, oxidative stress contributes to the development of kidney damage from the early stages. The levels of reactive oxygen species (ROS) increase after UUO, along with lipid peroxidation markers such as malondialdehyde (MDA). MDA is one of the free radical markers in the body and is the end product of lipid peroxidation, often used to assess oxidative stress in living cell membranes. DLBS 2411 is a bioactive fraction from Cinnamomum burmanii, an Indonesian native herb in tablet form. The chemical content of cinnamon possesses good antioxidant and anti-inflammatory properties. Methods A total of 30 male Sprague-Dawley rats were acclimatized for one week. MDA levels were then measured, followed by UUO surgery, and seven day post-UUO MDA levels were assessed. The rats were divided into 5 groups: Group 1 (Negative control), which received aquadest orally; Group 2 (Positive control), which was given methylprednisolone; Groups 3- 5 (UUO fibrosis model), which received DLBS 2411 cinnamon extract at doses of 50 mg/kg, 100 mg/kg, and 200 mg/kg body weight. After 14 days of treatment, MDA levels were re- examined. Results Paired t-tests showed that the negative control and positive control were ineffective in lowering MDA levels (p=0.232 and 0.265), while DLBS 2411 50, 100 and 200 mg/kgBB were effective in significantly reducing MDA levels (p=0.017; 0.007 and 0.009). Post hoc result, LSD test results indicated no significant difference in effectiveness between DLBS 2411 50 mg/kg and 200 mg/kg compared to the positive control in reducing MDA levels (p=0.100 and 0.063). However, at the 100 mg/kg dose, there was a significant difference in effectiveness in lowering MDA levels (p=0.035). Conclusion The 100 mg/kgBB dose of cinnamon extract DLBS 2411 is the most effective therapeutic dose in reducing MDA levels and preventing the progression of kidney fibrosis in male Sprague- Dawley rats with unilateral ureteral obstruction (UUO).
