Abstract

The Effect of Hemodynamically Significant Patent Ductus Arteriosus on Tissue Oxygenation in Preterm Infants at Mohammad Hoesin Hospital One of the most common congenital abnormalities of preterm birth is hemodynamically significant patent ductus arteriosus or hsPDA, the prevalence of which is about 70%. Clinical consequences are impaired tissue perfusion and oxygenation to organs. Near-Infrared Spectroscopy (NIRS) is a validated non invasive method for determining regional oxygen saturation (rSO2) that can identify early changes in organ perfusion and oxygenation. This study aims to determine whether there is an effect of hsPDA on the oxygenation of cerebral and renal regional tissues in preterm infants. A cross sectional study was conducted at Mohammad Hoesin Hospital. The study subjects were premature infants born in November 2023 - September 2024 who had been diagnosed with PDA and met the inclusion criteria of infants with chronological age > 24 hours and < 7 days and exclusion criteria of patients with multiple congenital anomalies, cyanotic PJB with ductal dependent, early onset sepsis and no parental consent. Statistical analysis using unpaired t test. The results of this study showed that the mean value of cerebral rSO2 in the hsPDA and non-hsPDA group were 79.53% and 80.99%, respectively. The mean value of renal rSO2 in the hsPDA and non-hsPDA group were 70.44% and 77.75%, respectively. The mean value of cerebral FTOE in the hsPDA and non-hsPDA group were 0.148. The mean value of renal FTOE in the hsPDA and non-hsPDA group was 0,36 and 0,186 respectively. No significant difference between two groups in rSO2 and FTOE in renal and cerebral.These findings suggest that preterm infant’s cerebral and renal tissue oxygenation is unaffected by the presence of hsPDA,. However, further research in preterm infants with gestational age ≤ 32 weeks and by looking at risk factors that affect the outcome of regional tissue oxygenation both cerebral and renal needs to be considered.
