Abstract
Background: Systemic Lupus Erythematosus (SLE) is an autoimmune disease affecting multiple organ systems. Incidence of children SLE increased by 15–17%, and outcomes are generally poorer compared to adults. Vitamin D plays an essen-tial role in immune regulation, and its deficiency may contribute to immune dysregulation in SLE. The Neutrophil-to-Lymphocyte Ratio (NLR) is a simple he-matological marker associated with systemic inflammation and disease activity in SLE. We aimed to evaluate the correlation between vitamin D levels, NLR, and disease activity measured by the SLE Disease Activity Index (SLEDAI) in children with SLE. Methods: A retrospective review of medical records was conducted for newly diagnosed pediatric SLE patients from January 2020 to December 2024. Results: Total of 144 children were included, with 97.2% aged over 10 years and 91.7% being female. Most patients had involvement of more than two organ sys-tems (85.4%) and infection comorbidities (39.6%). At diagnosis, 84% had high SLEDAI scores, 60.4% had vitamin D deficiency, and 53.6% had elevated NLR (≥3). The mean vitamin D level was 17.4 ± 5.3 ng/mL, and the average NLR was 3.3. Strong positive correlation was found between NLR and SLEDAI score (r = 0.89, p = 0.03). However, there was no significant correlation between vitamin D levels and either SLEDAI scores (r = 0.35, p = 0.21) or NLR (r = 0.17, p = 0.12). Conclusion: NLR is significantly correlated with disease activity in pediatric SLE, whereas vitamin D levels show no significant relationship with disease activity or NLR.
