Abstract

Pulmonary tuberculosis (TBC) remains a global health problem, with Indonesia ranking second in the world for the highest number of tuberculosis cases. Environmental factors, particularly housing physical conditions and smoking behavior, contribute to tuberculosis transmission, especially in densely populated areas such as the working area of Kertapati Public Health Center. This study aims to analyze the relationship between housing physical conditions (wall type, floor type, ventilation, lighting, temperature, humidity, and occupancy density) and smoking behavior with pulmonary tuberculosis incidence in the working area of Kertapati Public Health Center. This research employs a case-control study design with a quantitative approach, involving 112 respondents, consisting of 56 cases and 56 controls. Data were collected through interviews using questionnaires and direct observation of housing physical conditions, then analyzed using the chi-square test. The results showed a significant relationship between several housing physical condition variables and pulmonary tuberculosis incidence, including wall type (p= 0,014; OR= 2,78; CI 95%: 1,294-5,983), floor type (p= 0,014; OR= 2,78; CI 95%: 1,294-5,983), lighting (p= 0,000; OR= 4,92; CI 95%: 2,199-11,008), humidity (p= 0,023; OR= 2,57 CI 95%: 1,202-5,517), occupancy density (p= 0,002; OR= 3,52; CI 95%: 1,614-7,670), and temperature (p= 0,022; OR= 2,65; CI 95%: 1,220-5,744). However, ventilation and smoking behavior did not show a significant association with pulmonary tuberculosis incidence. Multivariate analysis showed that lighting was the dominant factor associated with pulmonary tuberculosis incidence, with a p-value= 0,0002 and OR= 5,62 (CI 95%: 1,880-16,779). In conclusion, houses with natural lighting of less than 60 lux increase the risk of developing pulmonary tuberculosis by 5,62 times compared to houses with adequate lighting.
