Abstract

Chronic Kidney Disease (CKD) is a condition of impaired kidney function that is influenced by clinical factors such as diabetes, hypertension, obesity, autoimmune, infection, or with other complications so that therapy involves polypharmacy. Polypharmacy therapy is thought to cause an increased risk of Drug Related Problems (DRPs). The purpose of this study was to analyze DRPs in CKD patients at RSI Siti Khadijah Palembang in 2025. The method used was a non-experimental descriptive method with a prospective cross-sectional approach. A total of 37 hospitalized CKD patients met the inclusion criteria. Analysis of DRPs according to the Pharmaceutical Care Network Europe (PCNE) V9.0 classification obtained 20 patients (54,06%) experiencing DRPs and 17 patients (45,94%) did not experience DRPs. DRPs that occurred were related to drug selection (C1) in 15 patients (40,55%), drug use process (C6) in 2 patients (5,4%) and relate to patient transfer (C8) in 3 patients (8,11%).
